  • Mobility, transfers & positioning
Safe use of walking aids, slide-boards, standing frames, hoists/slings; bed/chair transfers; posture support and comfortable positioning throughout the day and night.
  • Falls awareness & safety at home
Sensible layout adjustments, clear pathways, lighting checks, and simple prompts that reduce risks without reducing independence.
  • Pain & fatigue management
Pacing plans, joint protection, gentle stretches/relaxation as advised, heat/cold per guidance, and calm, energy-saving routines.
  • Skin integrity & pressure care
Turning schedules, pressure-relief cushions/mattresses, daily skin checks, and escalation for any redness or soreness.
  • Nutrition, hydration & mealtimes
Meal prep that matches preferences and any clinical needs; set-up for easier reach and safe, unhurried eating and drinking.
  • Medication & health oversight
Prompts or agreed administration, MAR records, noticing effects/side-effects, and liaison with GPs/therapists if something changes.
  • Therapy carry-over
Day-to-day support to practise physio/OT exercises, stretching and strengthening, orthoses/splint routines, and confidence with new equipment.
  • Access, community & work/study
Support for appointments, shopping, hobbies, sport, volunteering, education or employment—including transport planning and wheelchair-accessible journeys.
  • Assistive tech & adaptations
Set-up and daily use of chairs, beds, rails, grabbers, standing frames, environmental controls, and simple apps/timers—coordinated with OT.
  • Household help that actually helps
Laundry, light housekeeping, bed changes and tidy spaces that reduce effort and fall risks.
  • Family support & respite
Practical coaching (safe moving/handling), clear updates, and flexible breaks so relatives can rest.
